许智藏，高阳人。
北史卷九十 列传第七十八：
许智藏，高阳人也。祖道幼，常以母疾，遂览医方，因而究极，时号名医。诫诸子曰："为人子者，尝膳视药，不知方术，岂谓孝乎。"由是，遂世相传授。仕梁，位员外散骑侍郎。父景，武陵王谘议参军。智藏少以医术自达，仕陈，为散骑常侍。陈灭，隋文帝以为员外散骑侍郎，使诣扬州。会秦王俊有疾，上驰召之。俊夜梦其亡妃崔氏泣曰："本来相迎，如闻许智藏将至。其人若到，当必相苦，为之奈何？"明夜，俊又梦崔氏曰："妾得计矣，当入灵府中以避之。"及智藏至，为俊诊脉曰："疾已入心，即当发痫，不可救也。"果如言，俊数日而薨。上奇其妙，赍物百段。炀帝即位，智藏时致仕。帝每有苦，辄令中使就宅询访，或以辇迎入殿，扶登御床。智藏为方奏之，用无不效。卒于家，年八十。
宗人许澄，亦以医术显。澄父奭，仕梁，为中军长史，随柳仲礼入长安，与姚僧垣齐名，拜上仪同三司。澄有学识，传父业，尤尽其妙。历位尚药典御、谏议大夫，封贺川县伯。父子俱以艺术名重于周隋二代，史失其事，故附云。
