确字士潜，美容仪，宽大有行检，博涉经史，笃好玄言，世父弘正特所钟爱。解褐梁太学博士、司徒祭酒、晋安王主簿。高祖受禅，除尚书殿中郎，累迁安成王限内记室。高宗即位，授东宫通事舍人，丁母忧，去职。及欧阳纥平，起为中书舍人，命于广州慰劳，服阕，为太常卿。历太子中庶子、尚书左丞、太子家令，以父忧去职。寻起为贞威将军、吴令，确固辞不之官。至德元年，授太子左卫率、中书舍人，迁散骑常侍，加贞威将军、信州南平王府长史，行扬州事，为政平允，称为良吏。迁都官尚书。祯明初，遘疾、卒于官，时年五十九。诏赠散骑常侍、太常卿，官给丧事。
