史方，字正臣，开封人。应《周易》学究不中，补西第二班殿侍，再迁三班奉职，为潭、澧、鼎沿边同巡检，改右班殿直、阁门祗候。会澧州诉民下溪州蛮侵其土地，遣乘驿往视。自竹疏驿至申文崖，复地四百余里，得所掠五百余人，又置澧州、武口、杨泉、索溪四砦，以扼贼冲。就知邵州，徙澧州，迁右待禁。
天禧中，下溪州蛮彭仕汉寇辰州，杀巡检王文庆。方勒兵入溪洞讨捕，降其党李顺同等八百余人，诛其尤恶者社忽等十九人。迁西头供奉官、知辰州兼沿边溪洞都巡检使，修南、北江五砦，徙夔州。时富、顺州蛮田彦晏寇施州，焚暗利砦。方领兵直抵富、顺，荡其巢穴，穷追彦晏至七女栅，降之。迁内殿崇班，改内殿承制，奉使契丹，以供备库副使知环州、环庆路兵马都监。
先是，磨媚、浪（缺）、托校、拔新、兀二、兀三六族内寇，方谕以恩信，乃传箭牵羊乞和。减禁兵五千，徙内地以省边费。徙庆州，迁礼宾使兼环庆路兵马钤辖，复知环州。岁余，迁爱州刺史，为益州钤辖，徙秦凤路，迁西京作坊使，卒。
