刘式，袁州人。
宋史卷二百六十七 列传第二十六：
式字叔度，袁州人也。李煜时，举《三传》中第。归宋，历迁大理寺丞、赞善大夫、监通州丰利监及主三司都磨勘司，仍赐绯。式又建议置主辖支收司，以谨财赋出纳，时以为当。迁秘书丞，与陈靖使高丽。至道中，并三勾院为一，命式领之。再转工部员外郎，赐金紫。迁刑部。式深究簿领之弊，江、淮间旧有横赋，逋积至多，式奏免之，人以为便。然多所条奏，检校过峻，为下吏所讼，免官，卒。
真宗追录前效，赐其子立本学究出身。次子立之，后为国子博士。立德、立礼，并进士及第，立礼为殿中丞。
